A Brief Overview
The Heart Failure Program Coordinator provides professional, clinical nursing, and technical knowledge, skills, and expertise for the development and continuous improvement of comprehensive cardiac and/or heart failure patient care. Researches, develops, and applies evidence based practice in clinical care, patient and staff education, and the development and adherence of policies, procedures and treatment protocols, and overall plan of care. Provides data management oversight, data analysis, and reporting to identify patterns of care, benchmarked best practices, and opportunities for improvement, as well as concurrent reviews of the heart failure patients. Models appropriate behavior as exemplified in MLH Mission, Vision and Values.
What you will do
- Assessment: Provides clinical expertise to evaluate cardiac patients, heart failure protocol/processes, and associated data collection systems to support effective patient care delivery.
- Problem/Diagnosis: Coordinates with the healthcare staff to identify and diagnose patient care problems, implement approaches to these problems, and evaluate the effectiveness of the interventions.
- Coordinates the development and implementation of an educational program designed to convey specialized knowledge and skills to the healthcare staff, patients, families, and the community.
- Coordinates Cardiac Care Program goals using a multidisciplinary approach.
- Actively acts as a liaison for Emergency Medical Services to include collaboration, communication, education, process improvement and case reviews.
- Actively identifies, plans, and participates in research related to cardiac patients.
- Actively reviews day to day heart failure patients and communicates needs with the involved staff.
Education/Formal Training Requirements
- Required - Bachelor's Degree Nursing
Work Experience Requirements
- Required - Clinical nursing 5-7 years
Licenses and Certifications Requirements
- Required - BASIC LIFE SUPPORT - American Heart Association
- Required - Registered Nurse Tennessee - Tennessee Board of Nursing
Knowledge, Skills and Abilities
- Independent clinical decision-making skills.
- Ability to react calmly and effectively in emergency situations.
- Ability to evaluate and appropriately respond to verbal and nonverbal communication from patients with diverse states of development.
- Ability to coordinate and organize work.
- Teaching skills and ability to develop/mentor others.
- Experience with public speaking.
- Applied knowledge and experience utilizing tools/methods to perform clinical research and analysis in support of clinical quality Improvement activities.
- Ability to review and summarize data observations and findings, present the findings in a clear and concise way, and assist with decisions related to performance improvement opportunities.
- Ability to establish collaborative working relationships with patients, coworkers and community.
- Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals and ability to write routine reports and correspondence.
- Demonstrated proficiency in word processing, spreadsheets, and database programs. Experience with SPSS or Minitab preferred.
Supervision Provided by this Position
- There are no direct supervisory responsibilities. In a professional capacity, provides leadership for the Cardiovascular Program.
Physical Demands
- The physical activities of this position may include climbing, pushing, standing, hearing, walking, reaching, grasping, kneeling, stooping, and repetitive motion.
- Must have good balance and coordination.
- The physical requirements of this position are: medium work - exerting up to 50 lbs. of force occasionally and/or up to 25 lbs. of force frequently.
- The Associate is required to have close visual acuity including color, depth perception, and field of vision to perform an activity, such as assessing patients, preparing and analyzing data and figures; transcribing; viewing a computer terminal; or extensive reading.
- Frequent invasive and non-invasive patient contact.
- Exposure to patient body fluids as well as exposure to hazardous or poisonous materials.
- Ability to react quickly to emergency situations.
